Hospice Nurse Practitioner

We are seeking a highly skilled Hospice Nurse Practitioner to join our team at Gentiva Hospice. As a key member of our interdisciplinary team, you will be responsible for providing high-quality patient care and support to individuals and their families in need of hospice services.
Key Responsibilities:
* Perform Face-to-Face (F2F) encounters and gather clinical information to support hospice diagnosis and prognosis
* Communicate findings to the hospice physician responsible for certifying or recertifying patient eligibility
* Provide physical assessments and review medical treatment plans, diagnostic tests, lab results, and medication lists
* Make recommendations to the IDG based on F2F assessment findings
Requirements:
* Master's or higher degree in Nursing
* Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body
* Current state license as a registered nurse and advanced practice nurse
* Current national certification as a Nurse Practitioner (ANCC or AANP)
* Collaborative agreement in place
