### Job Summary

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team at the West Texas Rehabilitation Center. As a Hospice Nurse, you will provide high-quality patient care and support to individuals and their families in need of end-of-life care.

### Key Responsibilities

* Provide skilled nursing care to patients in their homes or in-patient settings
* Triage patient calls and respond to emergencies
* Supervise and mentor LVNs and aides
* Collaborate with interdisciplinary teams to develop and implement patient care plans
* Facilitate hospice transfers and notify hospital or skilled nursing facility admissions departments
* Provide emotional support and post-mortem care to patients and their families
* Educate patients and families on hospice care and services
* Document patient care and outcomes in a timely and accurate manner

### Requirements

* Current RN license in the State of Texas
* At least one year of experience as an RN
* Ability to travel and take after-hour calls
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Compassion and empathy for patients and families
