We are currently seeking an experienced Care Coordination Manager or Healthcare Transition Specialist to join our team. This role plays a crucial part in ensuring seamless transitions from skilled nursing facilities to home environments.

About the Role:

  • This position can be filled by either LPN or RN candidates who have at least 2 years of related nursing experience and a minimum of 3 years in a supervisory capacity.
  • The ideal candidate will have strong leadership abilities, comprehensive knowledge of healthcare transitions, and expertise in PPS, MDS, OBRA, and OSHA regulations.
  • You will be responsible for collaborating with skilled nursing communities, acute, and post-acute care providers to align resident goals with their assigned care plans.
  • As a Care Coordination Manager or Healthcare Transition Specialist, you will facilitate interdisciplinary communication and collaboration across multiple settings, ensuring effective care transitions and improved self-management skills among residents.
  • You will also assist the Director in managing a team of staff members, providing coaching, training, corrective action planning, goal setting, interviewing applicants, and conducting performance appraisals.

Key Responsibilities:

  • Coordinate the work of team members to meet resident goals and expectations.
  • Develop coaching relationships with residents and relevant stakeholders, empowering them to actively participate in their plan of care.
  • Follow up with required communication, support, and education to reduce the risk of readmission post-discharge.
  • Coordinate and evaluate care conferences, care plan updates, Med A meetings, discharge plans, and rehab/transition goals.
  • Evaluate resident readmissions and make recommendations for quality improvement practices.

What We're Looking For:

  • Registered Nurses or Licensed Practical Nurses in good standing, with a Bachelor of Science in Nursing preferred.
  • Strong leadership abilities, comprehensive knowledge of healthcare transitions, and expertise in relevant regulations.
  • Excellent customer service skills and ability to interact effectively with medical professionals, residents, family members, and care partners.
