Job Summary:

This position plays a critical role in ensuring seamless transitions for residents, fostering effective collaboration between skilled nursing facilities and post-acute care providers. As a Rehabilitation Transition Specialist, you will be responsible for coordinating care services to align with individual resident goals.

Key Responsibilities:

  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
  • Cultivate strong relationships with residents, families, and healthcare professionals to promote open communication and informed decision-making.
  • Facilitate education and support programs to empower residents and their loved ones to manage care effectively.
  • Evaluate care transitions and make recommendations for quality improvement initiatives.
  • Serve as a liaison between skilled nursing facilities and post-acute care providers to ensure continuity of care.

Requirements:

  • Registered Nurse or Licensed Practical Nurse in good standing required.
  • Bachelor of Science in Nursing preferred.
  • Minimum 2 years of related nursing experience, preferably in Senior Services.
  • Supervisory experience and knowledge of state and federal laws regulating skilled nursing facility operations essential.
