Job Summary

The Chronic Disease Case Manager provides comprehensive care and support to patients with chronic diseases, working closely with healthcare providers and other stakeholders to ensure seamless transitions of care and optimal patient outcomes.

Key Responsibilities
  • Develop and implement individualized care plans for patients with chronic diseases
  • Provide education and support to patients and their families on disease management, medication adherence, and healthy lifestyle choices
  • Collaborate with healthcare providers to coordinate care and ensure timely and effective communication
  • Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed
  • Provide referrals to community resources and services to support patient needs
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements
  • Associate's degree in a related field or equivalent experience
  • At least one year of experience in a medical facility
  • Current LPN license in the state of Arkansas
  • CPR and TB certification
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities
