Co-Team Lead Case Management Professional

Volunteers of America Chesapeake and Carolinas is seeking a skilled Co-Team Lead Case Management professional to join our team. As a key member of our Supportive Services for Veteran Families (SSVF) program, you will provide day-to-day management for SSVF staff, ensuring the effective delivery of services to homeless and low-income veterans and their families.
Responsibilities- Manage a caseload of SSVF program participants, providing case management, advocacy, and wraparound supports and services.
- Provide operational leadership for the SSVF program, including maintaining and keeping track of SSVF documents.
- Supervise and support SSVF case management teams, providing guidance and direction as needed.
- Coordinate client outreach and recruitment efforts, developing and implementing outreach plans to target participants as outlined in the SSVF program guidelines.
- Screen potential program participants for eligibility, verifying participant information and income status as needed.
- Conduct housing and comprehensive supportive services assessments for program participants, developing a comprehensive Family Services Plan.
- Provide service coordination between the program and community partners, ensuring participants receive services that support rapid re-housing and address assessed needs.
- Monitor and evaluate program effectiveness, analyzing data and preparing reports to inform program improvements.
- Bachelor's degree in counseling psychology, rehabilitative counseling, or a related human service field.
- Experience working with homeless individuals and families, including veterans, is a plus.
- Veteran or member of a veteran family is a plus.

- The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary.
- The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear, and perform basic and light home maintenance activities.
