Job Summary:

We are seeking a skilled and compassionate Registered Nurse to join our team at RHA Health Services. As a CAP Nurse, you will provide high-quality care to medically fragile children and their families, ensuring their health, safety, and well-being.

Responsibilities:
  • Establish eligibility for services and develop individualized plans to meet the unique needs of each client.
  • Conduct thorough assessments, interviews, and evaluations to identify client needs and develop effective care plans.
  • Collaborate with clients, families, medical providers, and community agencies to ensure comprehensive care and support.
  • Provide basic case management services, including coordination of services and referrals to other agencies as needed.
Requirements:
  • Graduation from an accredited School of Nursing and current RN licensure in North Carolina.
  • At least one year of nursing experience, preferably in a pediatric or home health setting.
  • Valid NC driver's license and a clean driving record.
