Key ResponsibilitiesThe Oncology Care Coordinator is responsible for ensuring the seamless coordination and continuity of care for patients undergoing various cancer treatments.
This role involves collaboration within a multidisciplinary team to oversee and manage all facets of care for a specific group of patients.
Responsibilities include assessing and planning in partnership with patients, families, and the healthcare team to address the cancer care needs throughout the care continuum.
Specific tasks may involve scheduling appointments across different specialties, serving as a liaison for patients and their families, delivering patient education, and identifying barriers to care.
Guidance will be provided to other healthcare staff as necessary. Care is delivered through a holistic, patient-centered approach, emphasizing the importance of assisting patients in accessing appropriate services and minimizing overall care costs.
The Oncology Care Coordinator will operate within the Mayo Clinic Nursing Professional Practice Model, which encompasses accountability for planning, executing, evaluating, and communicating all phases of nursing care for an assigned patient population.
This role promotes ethical decision-making and advocates for patients, enhancing their understanding of treatment plans and diagnoses. The Oncology Care Coordinator may also facilitate shared decision-making and collaboration among patients, caregivers, and the care team, while interacting with research professionals to promote awareness of clinical trial opportunities.
Qualifications- Graduate of a baccalaureate nursing program.
- Minimum of 2 years of clinical experience preferred.
- Experience in oncology preferred.
- Familiarity with disease population characteristics and desired outcomes preferred.
- Previous experience as a Nurse Navigator preferred.
Additional Qualifications:
- Proven leadership skills, effective communication, and strong critical thinking abilities are essential.
- Flexibility to work varied hours, including days, evenings, and weekends.
- Ability to adapt to unpredictable situations within the work environment is required.
Licensure or Certification:
- Current RN license in accordance with state regulations.
- Maintains Basic Life Support (BLS) competency.
- Must obtain relevant specialty certification within 2 years of hire.
