Patient Care Manager / Assistant Director of Nursing - Hospice

We are seeking a highly skilled and compassionate Registered Nurse to lead our hospice team. With a focus on delivering exceptional patient care, this dynamic role offers a competitive salary of $85,000 - $110,000 per year, comprehensive benefits, and opportunities for professional growth.

Key Responsibilities:
  • Supervise, mentor, and develop patient care staff to ensure high-quality care delivery.
  • Maintain compliance with agency policies, state and federal laws, regulations, and accreditation standards.
  • Participate in the manager-on-call rotation to provide 24/7 support.
  • Assist with talent acquisition, IDT meetings, quality assurance, and performance improvement efforts.
  • Manage patient case loads and facilitate IDT processes for multiple physicians and teams.
Requirements:
  • Minimum 3 years of hospice experience.
  • Valid RN Texas License.
  • CHPN certification preferred.
  • Valid Texas Driver's License and Proof of Auto Insurance.
