We are seeking a highly motivated and experienced Nurse Practitioner to join our interdisciplinary team at Evernorth Home-Based Care. As a Field Based Nurse Practitioner, you will be a critical part of our comprehensive care team, providing in-home primary care to patients with chronic, complex, and life-threatening illnesses.

Key Responsibilities:

Perform home visits with patients, conducting comprehensive evaluations and targeting risk factors for disease exacerbations and hospitalizations.

Prescribe medications, order labs or diagnostic testing, and make sub-specialty referrals, collaborating with the community PCP and other healthcare providers.

Educate patients and their families on chronic medical conditions, preventative care, and medication adherence compliance.

Coordinate with patient's other providers, including PCP, specialists, and health plan case managers, to ensure all are working together to achieve the same health goals.

Requirements:

Graduated from an Advanced Nurse Practitioner program in Adult, Geriatric, or Family Medicine.

Active, unrestricted NP license in Pennsylvania.

At least 1 year of NP experience in Primary Care, Geriatric, Adult, Home Care, or Family Medicine (new NP grads may be considered with the right RN experience).

Current BLS certification; ACLS preferred.

Ability to be credentialed by Medicare, Medicaid, and other Private Insurance Companies.

DEA license and State Controlled Substance License upon hire or within 3 months of hire.
