Registered Nurse Case Manager Hospice Home Care Specialist

About the Role
CarePartners is seeking a highly skilled Registered Nurse Case Manager to join our Hospice Home Care team. As a key member of our team, you will play a vital role in ensuring that our patients receive high-quality, patient-centered care.
Key Responsibilities
- Provide program orientation to patients, families, and caregivers, including an overview of the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting their strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
Requirements
- Basic Cardiac Life Support certification within 30 days of employment start date.
- Driver's License within 2 years of employment start date.
- Registered Nurse licensure within 2 years of employment start date.
- Associate Degree in Nursing.
