About the Role

As a LPN, you will visit patients in their homes and provide nursing care for them under the direction of RNs and in compliance with physicians' orders. Your responsibilities will include:

  • Providing nursing care to patients in their homes
  • Working under the direction of RNs and in compliance with physicians' orders
  • Developing and implementing individualized care plans for patients
  • Collaborating with other healthcare professionals to ensure comprehensive care
  • Documenting patient information and progress

Requirements

  • Valid State Driver's License
  • Automobile Liability Insurance
  • Dependable Transportation kept in good working condition
  • Ability to Drive in All Types of Weather Conditions
  • Current License as a Licensed Practical Nurse in the State of employment
