Job Summary

We are seeking an Advanced Practice Provider to join our team at Trellis Health, a start-up subsidiary of MaineHealth. As a key member of our care team, you will play a crucial role in shaping our clinical practices and patient experiences from the ground up.

Key Responsibilities
  • Manage a comprehensive patient panel, fostering trust-based relationships and proactive healthcare management.
  • Collaborate closely with other team members to develop, update, and prioritize patient care plans.
  • Ensure team-driven adherence to these plans, focusing on holistic health outcomes, patient satisfaction, and effective resource utilization.
Requirements
  • Successful completion of an approved Nurse Practitioner or Physician Assistant program.
  • Current nursing license issued by the State of Maine or eligible for licensure.
  • Minimum three years of professional experience, with two years of Nurse Practitioner or Physician Assistant experience preferred.
  • Strong digital literacy skills and proficiency with telehealth platforms.
  • Ability to maintain confidentiality and establish/maintain effective working relationships with patients, clinical staff, and the public.
