About the Role

This is an excellent opportunity for a part-time internal medicine (IM), family medicine (FM) or geriatric trained NP or PA to join our team. As a Post-Acute Advanced Practice Clinician, you will be responsible for delivering exceptional patient care and working collaboratively with our dynamic team to achieve outstanding patient outcomes.
