Key Responsibilities- Coordinate patient care across various settings, including pre-transplant, post-transplant, and living donor patients.
- Order and follow up on lab and imaging studies as per established protocols.
- Communicate test results to patients and healthcare providers, ensuring timely and accurate information exchange.
- Facilitate care management of defined patient populations, working closely with healthcare providers to ensure seamless care transitions.
- Coordinate information exchange between patients, healthcare providers, and other stakeholders to ensure comprehensive care.
- Graduate of an accredited Licensed Practical Nursing Program.
- Minimum 3 years of patient care experience.
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
