Key Responsibilities:- Provide primary and preventative care to patients, including health assessments, diagnoses, and treatment plans.
- Order and interpret diagnostic tests, prescribe medications, and implement interventions to support patient care.
- Collaborate with healthcare teams to coordinate patient care and make referrals as needed.
- Develop and maintain patient education plans to promote health and wellness.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Master's degree in Nursing or related field.
- Current APRN licensure and certification in Family Nurse Practitioner.
- Minimum 3 years of experience in Family Nurse Practitioner role.
- Strong computer skills, including electronic health records.
- Excellent communication and interpersonal skills.
