Join Our Team as a LPN Residential Nurse - ABI

We are seeking a dedicated and compassionate LPN to join our residential team as a LPN Residential Nurse - ABI. In this role, you will provide direct care to individuals with developmental disabilities and brain injuries, promoting their physical and emotional well-being. As a member of our interdisciplinary team, you will work closely with healthcare professionals, family members, and community partners to deliver person-centered care services.

Key Responsibilities

- Provide direct care to individuals with developmental disabilities and brain injuries, including personal care, medication management, and health education
- Develop and maintain relationships with healthcare providers, family members, and community partners to ensure seamless delivery of care services
- Collaborate with the multidisciplinary team to identify and address potential health risks and develop strategies to mitigate them

Requirements

- High School Diploma/GED required; College degree preferred
- Valid MA Nursing License and valid driver's license with acceptable driving record
- Strong communication and interpersonal skills, with ability to work effectively with diverse stakeholders
