Key Responsibilities

The Manager of Case Management will oversee the development and management of a comprehensive Case Management Program. This role includes providing leadership and guidance to case managers, social workers, discharge planners, and utilization review coordinators. The Manager will assess patient needs, design appropriate services, and ensure alignment with the hospital's mission and patient/family requirements. This position is also responsible for integrating services through continuous quality improvement methodologies, ensuring quality and resource management for all patients from admission through the continuum of care.

Qualifications

Required Qualifications:

  • Licensed clinician in the respective state.
  • Minimum of 5 years' experience in a Case Management role.
  • Analytical skills for problem identification and data evaluation.
  • Basic to intermediate computer proficiency.

Preferred Qualifications:

  • Certification in Case Management or a related field.
  • Current BCLS certification.
  • Familiarity with Milliman and InterQual Criteria.
