About the Role

The Veterans Affairs, Veterans Health Administration is seeking a highly skilled Nurse Care Manager/Coordinator to join our team. Responsibilities

- Develop and implement patient care plans that take into account the unique needs and goals of each patient
- Collaborate with interdisciplinary teams to ensure seamless care delivery and effective communication
- Provide education and support to patients and their families to promote self-management and prevention
- Lead a team of nurses in delivering high-quality patient care and mentoring new staff members
- Participate in quality improvement initiatives to enhance patient outcomes and experiences

Requirements

- A Bachelor's degree in Nursing (BSN) or Master's degree in Nursing (MSN)
- At least 2 years of experience in ambulatory care and leadership roles
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment and prioritize multiple tasks
- Strong analytical and problem-solving skills

- Competitive salary: $65,000 - $90,000 per annum
