Job Summary

We are seeking an experienced and compassionate LPN to work collaboratively with the RN case manager to provide skilled nursing services to patients in their homes. The ideal candidate will have strong clinical assessment skills, exceptional communication skills, and the ability to work within a team structure.

Responsibilities
  • Provide direct patient care and communicate changes in status to the physician and case manager.
  • Work collaboratively within the team structure and have strong communication skills.
  • Assess patients' needs and develop individualized care plans.
  • Provide education to patients and their families on disease management and self-care.
  • Document patient information accurately and thoroughly.
Requirements
  • Current Massachusetts Licensed Practical Nurse license in good standing.
  • 2+ years of LPN experience required.
  • Prior homecare experience preferred.
  • Strong clinical assessment skills and the ability to prioritize and triage patients.
  • Exceptional communication skills and the ability to work within a team structure.
  • Excellent organizational skills and time management skills.
  • Ability to work independently and understanding of how to access resources for patients.
  • Excellent computer skills.
