Job Title: Licensed Practical Nurse (LPN)

Aveanna Healthcare is seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in the comfort of their own homes. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Provide one-to-one patient care in a home environment, executing physicians' prescribed plans of care and documenting care provided in a timely manner.
  • Administer prescribed medication, treatments, and therapies, while promoting patient safety and well-being.
  • Conduct patient assessments, coordinate care, and educate patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
