About the Role

Pinnacle Home Care Inc. is seeking a skilled Physical Therapist Assistant to join our team of healthcare professionals. As a PTA, you will play a vital role in providing high-quality care to our patients in the comfort of their own homes.

Key Responsibilities
  • Assist in the evaluation and development of a rehabilitative plan of care, working closely with a Physical Therapist to ensure the best possible outcomes for our patients.
  • Participate in the preparation of clinical and daily progress notes, maintaining accurate and detailed records of patient care.
  • Perform routine strengthening and therapeutic exercises, and assist patients with the use of self-help devices, as directed by the Physical Therapist.
  • Teach exercise programs to patients and their families, promoting independence and self-care.
  • Identify potential issues with medication and report them to the Physical Therapist and physician.
  • Observe and record patient activities and findings, reporting any adverse reactions or concerns to the Physical Therapist and physician.
  • Consult with the Physical Therapist regarding patient outcomes and physical findings.
  • Collaborate with other healthcare team members, including Home Health Aides and family members, to ensure comprehensive care.
  • Write daily progress notes and submit them according to company policy, and prepare periodic summary reports as needed.
  • Participate in discharge activities and assist the Physical Therapist in completing the Physical Therapy Discharge Summary.
  • Confirm scheduling with the Supervisor/Director of Nursing to coordinate Agency visits.
  • Perform other duties as assigned by the Physical Therapist and Registered Nurse Case Management and Nursing Supervisor.
Requirements
  • Graduate of an accredited Physical Therapy Assistant program or equivalent education.
  • Licensed to practice as a PTA in the State of Florida.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Minimum of one year's experience in home healthcare preferred.
  • Electronic medical record experience is preferred but not essential.
  • Meets all qualifications as stated within the state practice act. Must possess and maintain current certification in cardiopulmonary resuscitation.
  • Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations.
  • Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
