Pinnacle Home Care Inc. is seeking a skilled and compassionate Physical Therapist Assistant (PTA) to join our team of healthcare professionals. As a PTA, you will play a vital role in providing high-quality patient care in the comfort of our patients' homes.
Key Responsibilities- Assist in the evaluation and development of a rehabilitative plan of care, working closely with a Physical Therapist (PT) to ensure effective treatment.
- Participate in the preparation of clinical and daily progress notes, ensuring accurate and timely documentation.
- Perform routine strengthening and therapeutic exercises, and assist patients with the use of self-help devices, as directed by the PT.
- Instruct patients and family members on exercise programs, promoting independence and self-care.
- Identify potential medication issues, adverse reactions, and contraindicated medications, reporting them to the PT and physician.
- Observe and record patient activities and findings, reporting any concerns to the PT and physician.
- Consult with the PT regarding home visit outcomes and report physical findings.
- Assist in teaching other healthcare team members, including Home Health Aides, on physical therapy techniques.
- Write daily progress notes and submit them according to company policy, and prepare periodic summary reports as needed.
- Participate in discharge activities, assisting the PT in completing the Physical Therapy Discharge Summary.
- Confirm visit scheduling with the Supervisor/Director of Nursing to coordinate Agency visits.
- Graduate of an accredited Physical Therapy Assistant program or equivalent education.
- Licensed to practice as a PTA in the State of Florida.
- Current BLS Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- Minimum of one year's experience in home healthcare preferred.
- Electronic medical record experience is preferred but not essential.
- Meets all qualifications as stated within the state practice act. Must possess and maintain current certification in cardiopulmonary resuscitation.
- Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations.
- Must be self-directed and able to work with little supervision, possessing good observational, judgment, and communication skills.
