Position Overview
The Practical Nursing Specialist provides essential nursing care in accordance with established care plans for patients facing terminal illnesses, ensuring compliance with the relevant Nurse Practice Act.

Key Responsibilities:
1. Deliver direct, skilled nursing care at the bedside.
2. Assess patient and family needs, contributing to the development and implementation of the interdisciplinary care plan.
3. Execute specific nursing tasks under the guidance of a registered nurse, including administering treatments and medications.
4. Monitor treatment effectiveness and patient responses, reporting significant changes to the registered nurse or team manager.
5. Maintain comprehensive clinical documentation and ensure a safe, clean environment for patients and their families.
6. Utilize a diverse range of nursing skills to fulfill medical orders, including medication administration and various treatments.
7. Collaborate with the interdisciplinary team to design and implement effective care plans.
8. Foster therapeutic relationships with patients and families, encouraging their active participation in care decisions.
9. Initiate nursing protocols tailored to the patient's physical and emotional needs, covering aspects such as mobility, hygiene, and communication.
10. Engage in patient and family education regarding illness management, daily living activities, and self-care practices.
11. Pursue ongoing professional development through in-service training and community educational opportunities.
12. Participate in on-call rotations as required to meet patient and family needs.

Qualifications:
1. Must possess reliable transportation for work purposes, along with necessary liability insurance and licensure.
2. Graduate from an accredited Practical/Vocational nursing program.
3. Current nursing license in the state of practice.
4. Minimum of one year of relevant nursing experience, including at least one year as an LPN/LVN in a medical-surgical unit.

Guidelines for Care Delivery:
1. Initial patient assessments are not conducted by this role.
2. Work is performed under the supervision of a registered nurse.
3. Reports to the designated Team Manager.

- Assist in lifting and transferring patients as needed.

- Perform tasks requiring close vision, distance vision, and the ability to adjust focus.
