Role Responsibilities
Under the supervision of a Physical Therapist (PT), the Rehabilitation Therapy Assistant will provide client services as delegated by the PT.
- Assist in evaluating and developing rehabilitative care plans and conduct periodic re-evaluations as necessary.
- Participate in the creation of clinical and daily progress documentation, which will be reviewed by the PT and submitted in accordance with company policies.
- Perform routine strengthening and therapeutic exercises as directed, and assist patients with self-help devices.
- Collaborate with the PT to instruct clients and their families on exercise programs.
- Identify potential ineffective drug therapies or adverse reactions, significant side effects, drug allergies, and contraindicated medications, reporting these to the PT.
- Observe and document activities and findings in clinical records, reporting any adverse reactions to treatment to the PT and physician.
- Consult with the PT regarding the outcomes of home visits and communicate physical findings.
- Instruct other health team members, including Home Health Aides and family members, on specific aspects of physical therapy to facilitate client care.
- Prepare daily progress notes on patients and submit them according to company policy, along with periodic summary reports as needed.
- Engage with staff, clients, and physicians in discharge planning and assist the Physical Therapist in completing discharge summaries.
- Confirm visit schedules weekly with the Supervisor/Director of Nursing to coordinate agency visits.
- Assume additional responsibilities as deemed necessary by the PT and designated by the Registered Nurse Case Management and Nursing Supervisor.
Qualifications
- Graduate of an accredited Physical Therapy Assistant program or equivalent education.
- Licensed to practice as a PTA in the State of Florida.
- Current BLS Certification.
- Licensed driver with a reliable automobile that meets the organization's insurance requirements.
- Preferred: Minimum of one year’s experience in home healthcare.
- Preferred: Experience with electronic medical records, though not essential.
- Must meet all qualifications as outlined in the state practice act and maintain current CPR certification.
Applicants should demonstrate good emotional health and the ability to physically manage standing, stooping, bending, and heavy lifting, adhering to all agency regulations. Self-direction and the ability to work with minimal supervision, along with strong observational, judgment, and communication skills, are essential.
