Position Overview:

The Surgery/Trauma Clinical Pharmacist plays a vital role within a multidisciplinary healthcare team, delivering comprehensive pharmaceutical services to patients with high healthcare needs at Vanderbilt University Medical Center. This role primarily focuses on supporting trauma care teams and assisting nursing staff on surgical stepdown units. Key responsibilities include participating in patient rounds, establishing pharmacotherapeutic objectives, recommending treatment regimens, and creating monitoring plans tailored to individual patient needs. The clinical pharmacist will also provide deprescribing recommendations and facilitate seamless transitions of care. Additionally, this position involves leading initiatives to prevent and report medication errors and adverse drug reactions, in accordance with institutional policies, while continuously enhancing medication safety practices.

Key Responsibilities:

  • Evaluates medication orders and prescriptions to ensure safe, effective, and economical medication use. Prepares and dispenses medications, provides drug information, and manages medication therapy in compliance with regulations and hospital policies.
  • Delivers exceptional patient care by optimizing medication management and resolving medication-related issues.
  • Maintains high standards of quality, safety, and consistency in clinical pharmacy services across designated areas.
  • Supervises the activities of support personnel, ensuring the accuracy and appropriateness of all medications prepared prior to patient dispensing.
  • Additional responsibilities may be assigned as necessary.

Qualifications:

Preferred:

  • Completion of an ASHP-accredited PGY1 residency plus over a year of relevant experience, or completion of an ASHP-accredited PGY2 residency (in internal medicine, geriatrics, or a related field).
  • Board of Pharmacy Specialties Certification in Geriatrics (BCGP) or Pharmacotherapy (BCPS).
  • Experience in an academic medical center setting.

Work Schedule:

  • Day Shift
  • Every 4th weekend
  • Holiday rotation
