Home Health Care Physical Therapist Assistant

Pinnacle Home Care Inc. is seeking a highly skilled and compassionate Physical Therapist Assistant to join our team of healthcare professionals. As a Home Health Care Physical Therapist Assistant, you will play a vital role in delivering exceptional patient care and support to individuals in the comfort of their own homes.
Key Responsibilities- Assessment and Treatment Planning: Assist in the evaluation and development of a rehabilitative plan of care, working closely with our Physical Therapists to ensure comprehensive and effective treatment.
- Patient Education and Support: Instruct patients and their families on exercise programs, self-help devices, and other aspects of physical therapy, promoting independence and empowerment.
- Documentation and Communication: Maintain accurate and detailed records of patient progress, communicating effectively with our healthcare team and patients' physicians to ensure seamless care.
- Collaboration and Teamwork: Work collaboratively with our multidisciplinary team, including Physical Therapists, Home Health Aides, and other healthcare professionals to provide comprehensive care.
- Education and Licensure: Graduate of an accredited Physical Therapy Assistant program and licensed to practice as a PTA in the State of Florida.
- Experience and Skills: Minimum of one year's experience in home healthcare, with excellent observational, judgment, and communication skills.
- Certifications and Training: Current CPR certification and electronic medical record experience preferred.
