Job Summary

We are seeking a skilled Patient Care Coordinator to join our team in the Pediatric and Adult Unit, working evening shifts on a part-time basis.

Key Responsibilities
  • Anticipate and accommodate patient needs by responding to their verbal and nonverbal cues.
  • Prepare patients for daily therapy sessions, ensuring they are comfortable and ready for treatment.
  • Provide patients with opportunities for independence and decision-making, aligned with their care plan.
  • Assist with patient admission, transportation, and discharge, ensuring a smooth transition.
  • Aid patients with daily living activities, including dressing, grooming, and personal hygiene.
  • Recognize changes in patient status and report relevant information to registered nurses.
  • Maintain patient living areas, ensuring a clean and comfortable environment.
  • Protect patient rights, privacy, and confidentiality, and educate patients and families on these rights.
Requirements
  • High school diploma or equivalent (required)
  • Basic Life Support (BLS) certification, accredited by the American Heart Association (AHA)
  • Experience in a healthcare setting (preferred)
  • State Registered Nurse Aide (SRNA) certification by the Kentucky Board of Nursing (preferred)
