About the Role

We are seeking a dedicated and compassionate Patient Care Coordinator to join our team at UofL Health - Frazier Rehabilitation Institute.

Key Responsibilities
  • Anticipate and accommodate patient needs by responding to their verbal and nonverbal cues.
  • Prepare patients for daily therapy sessions, ensuring a smooth and efficient experience.
  • Provide patients with opportunities for independence and decision-making, aligned with their care plan.
  • Assist with patient admission, transportation, and discharge, ensuring a seamless transition.
  • Aid patients with daily living activities, including transferring, repositioning, and ambulation.
  • Recognize changes in patient status and report relevant information to registered nurses.
  • Maintain patient living areas, ensuring a clean and comfortable environment.
  • Protect patient rights, privacy, and confidentiality, and educate patients and families on these rights.
Requirements
  • High school diploma or equivalent (required)
  • Basic Life Support (BLS) certification accredited by the American Heart Association (AHA)
  • Preferred qualifications: experience in a healthcare setting, State Registered Nurse Aide (SRNA) certification by the Kentucky Board of Nursing
