Job Summary

We are seeking a Patient Care Coordinator to join our team. The successful candidate will perform clerical duties and clinical patient care support on the unit under the direction of licensed nursing personnel. This role requires strong communication skills, attention to detail, and the ability to work effectively in a fast-paced environment.

Key Responsibilities
  • Transcribe and compute physician orders, department orders, and messages accurately
  • Assemble patient medical records and maintain proper order of records within the chart
  • Set up admission charts and disassemble discharged and transferred charts, checking for completeness
  • Handle desk communications properly and promptly, including responding immediately to intercom, pages, and routing of messages to appropriate personnel
  • Replace forms as necessary in patient medical records
  • Route requests for patient information to nurse promptly and accurately
  • Communicate physician orders to responsible nurse
  • Collect required unit data
  • Measure and document intake and output, heights and weights, and vital signs
  • Apply, monitor, and document restraint usage
  • Collect urine, stool, and sputum specimens
  • Perform and document point-of-care testing
  • Perform and document patient mobility and exercise, and comfort, rest, and sleep care needs
  • Perform and document patient hygiene, grooming, and dressing, skin and nail care, dietary, and toileting and elimination needs
Requirements
  • High school diploma or GED/equivalent
  • Basic Life Support (BLS) accredited by the American Heart Association (AHA)
  • Experience in a healthcare setting (preferred)
  • State Registered Nurse Aide (SRNA) by the Kentucky Board of Nursing (preferred)
