Job Summary:

Native American Connections is seeking a highly skilled and detail-oriented Intake Coordinator to join our team. As an Intake Coordinator, you will be responsible for providing comprehensive coordination for smooth referral, assessment, and admission processes for clients entering NAC programs. This includes eligibility verification, referral review, assessment, prior authorization submission if needed, and admission into programming.

Key Responsibilities:

  • Provide excellent customer service to clients and referring agencies.
  • Receive and acknowledge incoming referrals (by fax, email, and phone calls/walk-ins) while reviewing payor/insurance eligibility and creating client file in EHR within 24 business hours.
  • Screen referrals for necessary clinical information and schedule clients for assessments as quickly as possible (same day if possible).
  • Check client eligibility regularly and submit for initial prior authorization for residential admissions requiring prior authorization.
  • Notify clients and referral sources of all documents and needs for entering treatment.
  • Assist with coordination of detox and/or MAT services as needed throughout referral, assessment, and/or admission process.
  • Complete assessments, integrated service plans, and related documentation. Recommend appropriate services for clients based on assessment process. Referrals made to internal programs as well as external providers.
  • Prioritize priority populations (i.e., Native American, pregnant, IV substance use, etc.) for residential admission on an ongoing basis to ensure high bed occupancy.
  • Follow up on all authorizations to ensure clients are not in residential treatment without proper authorization.
  • Coordinate and complete residential admissions as needed and keep the admissions center staff informed of client admissions schedules, status, and related issues.
  • Maintain strong working and referral relationships with all existing contractors and collaborators, potential contractors, and other numerous community and referral agencies.
  • Attend meetings as needed to be fully aware of authorization and concurrent review processes.
  • Assist in making community referrals and support as needed.
  • Support and actively engage in practices of inclusion through Cultural and Linguistically Appropriate Services (CLAS) Standards.
Requirements:

  • Bachelor's degree in behavioral health field with 2 years' experience or Associates of Arts Degree in behavioral health field and 4 years' experience (Behavioral Health Technician).
  • MS Office proficiency.
  • Strong interpersonal skills with an ability to work effectively with a wide range of people, teams, managers, supervisors, and vendors.
  • Demonstrate excellent written and oral communication skills.
  • Excellent customer service skills.
  • Experience in working with the Native American population preferred.
  • Must demonstrate critical thinking, problem-solving, and organizational and time management skills.
  • Proficiency in electronic health record systems.
  • Must be able to work well with others in a team approach.
  • Possess and maintain a valid Arizona driver's license.
  • Fingerprint Certificates within 90 days of hire.
  • Bilingual-Spanish desired.
  • Understand the dynamics of substance use and demonstrate the ability to integrate the knowledge for practical application.
  • Must understand the recovery process of a substance-using individual.
  • Know the economic, educational, and social problems of Native Americans and referral sources available.
