As a key member of our interdisciplinary team, you will be responsible for delivering high-quality, patient-centered care that addresses the unique needs of individuals with mental health conditions.
Responsibilities- Patient Assessment: Conduct thorough and ongoing holistic nursing assessments to identify patient needs and concerns, and develop individualized plans of care that address these needs.
- Patient Care/Discharge Planning: Collaborate with patients, families, and healthcare team members to coordinate care, develop discharge plans, and ensure seamless transitions of care.
- Teaching and Education: Provide individualized teaching and education to patients and families to promote health and wellness, and empower them to manage their mental health conditions.
- Communication and Collaboration: Demonstrate effective communication and collaboration skills to work with interdisciplinary teams, families, and community resources to ensure comprehensive care.
- Treatment and Procedures: Perform treatments and procedures in accordance with established policies and procedures, and collaborate with healthcare team members to ensure safe and effective care.
- Documentation and Quality Improvement: Accurately document patient information, and participate in quality improvement initiatives to enhance patient outcomes and improve processes.
- Education: Graduate of an accredited school of nursing, currently licensed in the State of New York, or permittee.
- Experience: Basic knowledge of nursing theory and practice, and experience working with patients with mental health conditions.
- Licenses/Certifications: New York State Registered Nurse license required, or permittee.
