Job Summary:

We are seeking a compassionate and skilled Registered Nurse (RN) to join our care team at Hope Community Support Services. As a Nurse Care Coordinator, you will play a vital role in providing direct care, monitoring residents' health and wellness, and collaborating with staff to ensure the highest standard of care.

Key Responsibilities:
  • Conduct regular health assessments and respond to residents' medical needs.
  • Oversee prescribed medications administration and treatments, track and document residents' health status, and update care plans as necessary.
  • Initiate and lead interventions that improve health outcomes for participants, educate residents and their families, and contribute as a member of a multidisciplinary team.
  • Educate resident, families, and caregivers on health, wellness, safety, self-management, medication, self-monitoring, action plans, long-term planning, and accessing care.
Qualifications:
  • Registered Nurse (RN) with a valid state license in Maine.
  • CPR and First Aid certification.
  • Experience in mental health, residential, or community health settings is preferred.
  • Strong interpersonal and communication skills with a compassionate, resident-centered approach.
  • Familiarity with electronic health record (EHR) systems and Microsoft Office Suite.
