Estimated Salary: $60,000 - $80,000 per year

The ideal candidate will possess excellent communication skills, a strong understanding of psychiatric care, and the ability to work effectively in a fast-paced environment. Our hospital's Performance Improvement Plan relies on collaboration among multidisciplinary teams to drive quality improvement initiatives. As a Care Coordinator Specialist, you will actively participate in these efforts and contribute to ongoing continuing education programs.

Job Responsibilities:

  1. Patient Support: Collaborate with unit treatment teams to identify specific discharge planning needs for each patient, ensuring seamless transitions from hospital care to community-based services.
  2. Resource Management: Develop and maintain comprehensive knowledge of agencies, organizations, and resources within the referral community, providing patients with access to necessary support systems.
  3. Case Management: Assist assigned social workers in securing housing, completing Maine Care applications, and facilitating case management or other community supports as needed.
  4. Liaison Role: Foster reciprocal relations with county providers by serving as a liaison from the hospital and making regular visits to various community programs when applicable.

Requirements:

  • Education: Bachelor's Degree in a related field preferred.
  • License/Certifications: LSW, LSW Conditional, MHRT-C, or Provisional MHRT/C required.
  • Experience: Five years of relevant experience in healthcare, with a psychiatric setting preferred.
