Advanced Practice Clinician, Integrated Care Specialist

We are seeking an experienced Advanced Practice Clinician to join our team as an Integrated Care Specialist. This role will provide primary care to SCAN members, working closely with our case management team to ensure timely access to necessary services.
Key Responsibilities- Provide care to an assigned panel of members, conducting physical exams, administering vaccines, and managing chronic diseases.
- Identify patient health care needs and develop comprehensive plans of care, collaborating with our case management team.
- Manage visitation schedules, adjust based on member needs, and engage with Primary Care Physicians for clinical oversight.
- Connect with specialists, hospitals, and community-based organizations to ensure continuity of care and promote shared decision-making.
- Document member visits and interactions in a timely manner, serving as a direct resource for members and their families.
- Master's degree in Nurse Practitioner Certificate or Master's from an accredited PA program.
- Current California Nurse Practitioner license or California Physician Assistant Certification (PA-C) in good standing.
- Knowledge of complex chronic diseases, such as dementia, diabetes, heart failure, and COPD.
- Ability to identify health care needs and refer and coordinate with other resources/programs.
- Base salary range: $125,400.00 to $200,600.00 annually.
