Advanced Practice Clinician

Homebase Medical is seeking an experienced Advanced Practice Clinician to join our team in San Jose, CA. As a key member of our healthcare team, you will be responsible for conducting Medicare Annual Wellness Visits in our partner clinic.
Responsibilities:- Perform Medicare Annual Wellness Visits in our partner clinic.
- Review patient medical and family history.
- Review current providers and prescriptions.
- Perform height, weight, blood pressure, and other routine measurements.
- Deliver personalized health advice.
- Review risk factors and potential treatment options.
- Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
- Provide health education to patients.
- Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
- Provide referrals to appropriate medical resources.
- Utilize our EMR system to document patient visit/interaction accurately and timely.
- Knowledge of HCC coding.
- Other duties as assigned.
- Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
- Current, unrestricted PA or NP license to practice in California including DEA license.
- Minimum of 3 years of experience in primary care, hospice, or urgent care/outpatient setting.
- Fluent in Vietnamese, required.
- Minimum of 1 year of previous experience utilizing an EMR system.
- Good working knowledge of HCC coding.
- Strong computer skills including MS Office and Outlook is strongly desired.
- Great verbal and written communication skills.
- Current drivers' license, auto insurance and reliable transportation.
- Compassionate and caring attitude with a desire to live our mission, vision and values.
- PayScale information: $75.00 - $100.00 per hour.
