Job Title: Advanced Practice Clinician

Homebase Medical is a leading provider of home-based medical services, dedicated to improving the quality of life for homebound older adults. We are seeking an experienced Advanced Practice Clinician to join our team in the San Jose, CA area.

Responsibilities:
  • Conduct Medicare Annual Wellness Visits in our partner clinic.
  • Review patient medical and family history.
  • Review current providers and prescriptions.
  • Perform height, weight, blood pressure, and other routine measurements.
  • Deliver personalized health advice.
  • Review risk factors and potential treatment options.
  • Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
  • Provide health education to patients.
  • Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
  • Provide referrals to appropriate medical resources.
  • Utilize our EMR system to document patient visit/interaction accurately and timely.
  • Knowledge of HCC coding.
  • Other duties as assigned.
Qualifications and Experience:
  • Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
  • Current, unrestricted PA or NP license to practice in California including DEA license.
  • Minimum of 3 years of experience in primary care, hospice, or urgent care/outpatient setting.
  • Fluent in Vietnamese, required.
  • Minimum of 1 year of previous experience utilizing an EMR system.
  • Good working knowledge of HCC coding.
  • Strong computer skills including MS Office and Outlook is strongly desired.
  • Great verbal and written communication skills.
  • Current drivers' license, auto insurance and reliable transportation.
