Job Summary:

Children's Aid is seeking a highly skilled Case Planner to join our team in Brooklyn. As a Case Planner, you will be responsible for providing culturally competent, short-term Crisis Intervention treatment to families seeking Family Stabilization services in the boroughs of the Bronx, Manhattan, Brooklyn, and Staten Island.

Key Responsibilities:

• Provide short-term Crisis Intervention treatment to families in need

• Conduct home assessments and assessments of financial resources for families

• Assist families with concrete needs and services, educational advocacy, and referrals to support services to alleviate barriers to their participation in treatment

• Collaborate with Educational and Employment specialist to monitor school attendance, obtain school records, and intervene with DOE when appropriate to develop an appropriate education/vocational plan for youth

• Research and identify appropriate community-based services to support families, make referrals to community-based organizations/agencies as needed, and engage in outreach activities in the community

Requirements:

• Minimum Bachelor's Degree in Social Work or related field required

• Experience working with adolescent and/or families preferably in a child welfare, juvenile justice, or mental health setting

• Bilingual in English and Spanish preferred

• Good organizational skills with ability to meet deadlines

• Ability and willingness to work with families and teens in the home, school, and other appropriate settings in the community

• Ability to work flexible hours including evenings and some weekends as needed

• Ability to work independently, as well as contribute to a team

• Strong clinical skills and ability to work under pressure with high-risk, crisis-oriented cases

• Very good interpersonal/communication skills, including ability to write well and meet copious amounts of documentation in a timely manner

• Computer literate
