Key Responsibilities:

  • Assess and identify the ongoing needs of residents to ensure the highest standard of care.
  • Participate in evaluations and documentation for residents experiencing changes in health status.
  • Conduct thorough wellness evaluations for all residents on a regular basis.
  • Monitor incidents involving residents and ensure proper documentation and follow-up.
  • Maintain open communication with residents and their families regarding medical needs.
  • Ensure compliance with all health regulations and standards.
  • Support community team members with clinical guidance and expertise.
  • Oversee medication administration and treatment protocols to ensure accuracy and compliance.

Qualifications:

  • Graduate from an accredited nursing program.
  • Current state licensure as a Licensed Practical Nurse (LPN).
  • At least one year of experience in a healthcare setting, such as home health or assisted living.
  • Strong assessment and communication skills.
  • Ability to manage multiple priorities effectively.
  • Proficient in Microsoft Office and healthcare applications.
  • Willingness to work flexible hours, including weekends and evenings.
