Job Summary

We are seeking a skilled Case Manager to join our team at Family Service Association. As a Case Manager, you will be responsible for providing high-quality support services to adult clients in need of guardianship assistance.

Key Responsibilities
  • Conduct regular face-to-face visits with clients at their place of residence to assess their needs and provide support.
  • Complete monthly progress notes and documentation of collateral contacts to ensure accurate record-keeping.
  • Provide transportation for client visits and participate in occasional travel to Fall River for supervision or staff meetings.
  • Ensure confidentiality and respect the sensitive nature of program services and communications.
  • Collaborate with fiscal department to ensure clients on MassHealth stay within income guidelines.
  • Participate in on-call rotation to respond to emergency situations during weeknights and weekends.
Requirements
  • Bachelor's degree in Psychology, Social Work, Sociology, or related field.
  • Experience working with disabled or elderly adults in a community or long-term care setting.
  • Experience with fiscal accounting or management of client funds.
  • Case management experience ensuring continuity of care.
