Case Manager

• Conduct regular face-to-face visits with assigned adult clients at their place of residence within the state of Massachusetts.
• Complete monthly progress notes and documentation of collateral contacts.
• Provide reliable transportation for client visits and occasional travel to Fall River for supervision or staff meetings.
• Maintain the confidential nature of all program services and communications.
• Ensure all necessary consents have been given prior to scheduled procedures for clients' medical and psychiatric care.
• Participate in on-call rotation, responding to emergency situations as needed.
• Collaborate with the fiscal department to ensure clients on MassHealth stay within income guidelines set forth by the state of Massachusetts.
• Report overdrawn and over-asset accounts to the Guardianship/Rep Payee Financial Supervisor and take appropriate action to remedy problems.
• Assist with the management of clients' funds and ensure their financial needs are met.
Education and Experience:
• Bachelor's degree in Psychology, Social Work, Sociology, or a related field, or commensurate experience and prior experience working with disabled/elderly adults in a community and/or long-term care setting.
• Experience with fiscal accounting or management of clients' funds.
• Experience with case management, ensuring continuity of care.
