Job Summary

CenterWell Home Health is seeking a skilled and compassionate LPN to join our team as a Home Health LPN. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver comprehensive care.

Key Responsibilities
  • Provide skilled nursing services under the direction of a RN, adhering to state and federal regulations, and CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans, utilizing a holistic approach to address patients' physical, emotional, and social needs.
  • Confer with RN Supervisors to ensure seamless care coordination and make necessary adjustments to patient plans.
  • Maintain accurate and detailed documentation, adhering to company standards and regulatory requirements.
  • Collaborate with patients, families, and healthcare teams to ensure effective communication and care delivery.
Requirements
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid drivers license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
