LPN Home Health Part Time

CenterWell Home Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality home health care to patients in need. As an LPN, you will play a vital role in delivering compassionate and personalized care to our patients, working closely with our interdisciplinary team to ensure seamless care coordination and exceptional patient outcomes.
Key Responsibilities- Provide skilled nursing services to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to address these needs.
- Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and optimal patient outcomes.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and address any concerns or questions.
- Document patient care and progress in a timely and accurate manner, adhering to regulatory and company standards.
- Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
At CenterWell Home Health, we offer a competitive compensation package, including a base salary range of $44,200 - $60,900 per year, as well as opportunities for professional growth and development.
