We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will provide high-quality nursing care to patients in their homes, helping them to recover from illness, injury, or surgery.
Key Responsibilities- Provide professional nursing services under the direction of a RN, in compliance with state and federal regulations, and CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
