Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As an LPN with CenterWell Home Health, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their health goals.

Responsibilities
  • Provide skilled nursing interventions to patients in their homes, including wound care, medication management, and education on disease management.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to address these needs.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
  • Current LPN licensure in the state of practice.
  • Minimum of 2 years of experience as an LPN in a home health or hospice setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on providing high-quality patient care.
