Job Summary:

First Connect is seeking a highly skilled Physical Therapy Assistant to join our team. The successful candidate will provide exceptional care and support to patients in a long-term care setting, working under the supervision of a licensed Physical Therapist.

Responsibilities:

  • Conduct physical therapy sessions and develop individualized treatment plans to meet patient needs
  • Collaborate with healthcare teams to ensure comprehensive patient care
  • Document patient progress and maintain accurate records

Requirements:

  • Bachelor's degree in Physical Therapy or a related field
  • Current state licensure as a Physical Therapy Assistant
  • 1+ year of experience in a long-term care setting (SNF experience preferred)
  • Covid-19 vaccination required

Compensation and Benefits:

  • A competitive salary of $62,000 - $72,000 per annum, based on experience and qualifications
