Assignment Overview: This position is for a Physical Therapist to work in a LTC or SNF setting, providing high-quality patient care and services.

Key Responsibilities:

  • Patient Care: Assess, plan, and implement individualized physical therapy programs for patients.
  • Communication: Collaborate effectively with interdisciplinary teams, patients, and families to ensure seamless care delivery.
  • Documentation: Maintain accurate and thorough records of patient progress and treatment plans.

Requirements:

  • Education: Master's degree in Physical Therapy from an accredited program.
  • Licensure: Current licensure as a Physical Therapist in the state where the assignment is located.
  • Certifications: BLS certification and any other required certifications for the position.
