About the Role:

We are currently seeking a qualified Physical Therapy Assistant to join our team at First Connect. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support in a long-term care setting.

Key Responsibilities:

  • Conduct physical therapy sessions under the supervision of a licensed Physical Therapist
  • Develop and implement individualized treatment plans to meet patient needs
  • Collaborate with healthcare teams to ensure comprehensive patient care
  • Document patient progress and maintain accurate records

Requirements:

  • Bachelor's degree in Physical Therapy or a related field
  • Current state licensure as a Physical Therapy Assistant
  • 1+ year of experience in a long-term care setting (SNF experience preferred)
  • Covid-19 vaccination required
