Job Description

We are seeking a highly skilled and compassionate Occupational Therapist to join our home health care team as a Home Health Care Manager OT. In this role, you will be responsible for assessing patients' needs, developing and implementing individualized care plans, and providing education to patients and families.

You will work closely with our multidisciplinary team to ensure that patients receive comprehensive and coordinated care. Your expertise in occupational therapy will enable you to identify opportunities for improvement and develop strategies to enhance patient outcomes.

Requirements:

  • Master's degree in occupational therapy
