Physical Therapist for Home Visits

CenterWell Home Health is seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and functional disabilities in the comfort of their own homes.
Key Responsibilities- Plan and administer prescribed physical therapy treatment and training for patients to achieve maximum physical function.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function and develop treatment programs.
- Develop and implement conditioning and rehabilitation programs consistent with physicians' plans of treatment.
- Confer with physicians and clinical team members to obtain additional patient information and develop treatment plans.
- Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aides, and supervise their performance.
- Accurately document patients' care observations, interventions, and evaluations.
- Report patients' progress to physicians, Clinical Managers, staff, patients, and families.
- Degree from an accredited Physical Therapy Program.
- Minimum of one year physical therapy experience preferred.
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Strong organizational and communication skills.
